Awkwardly enough, those extremely realistic vaginas featured in The Witcher 3's next-gen upgrade were apparently accidentally added.
How one accidentally adds a vagina where there were none is a mystery that we may never solve. Even Frankenstein didn't make mistakes like this. In the original version of The Witcher 3, female characters had no genitalia at all, like a Barbie doll. Yet, the next-gen version of the beloved RPG brings all sorts of improvements, including “fully integrated fan-made mods including 4K textures and models.” From the sounds of things, CD Projekt Red is as taken aback as players were to see... well... that.

“The next-gen version of The Witcher 3: Wild Hunt features several community-sourced mods not created by CD Projekt Red, on top of numerous enhancements created and implemented by the studio internally. Merging everything together was a complex process and the textures in question are an unintended result present in the release version. This is something we are working to address,” said the developer in a statement to Kotaku, specifically mentioning that the realistic vaginas “were not meant to be present in the release version of the game.”

Again, I'm still unconvinced that this mod made it into that collection with no one noticing, and there's odd wording in that sentence. The textures were present in the development version of the game, but I can't think of a reason why this specific mod was needed for internal testing.
Moreover, the mod only affects the crones from Crookback Bog and the bruxas. This is a safe space, up until a point. At the moment there is no confirmed date for the update that removes the genitalia, but I think I speak for the majority when I say I hope it's soon.
