One Fallout 3 player wanted to see what would happen if they killed every single NPC and enemy in the game and embarked on an impressive killing spree.
The goal of the playthrough was simple, Nerbit would kill every NPC who wasn’t valuable, as in needed for a quest, but everyone else was fair game.

They also made sure that any NPC who was needed for a quest would be killed once that quest was over.

Nerbit has previously completed very similar runs of Fallout 4 and Fallout: New Vegas, both of which totalled up many, many kills and needed some mods to act out the plan.
By the end of the run, Nerbit had stacked up some impressive stats.
They’d completed 41 quests, visiting 107 locations. An impressive 67 locks were picked, while 29 computers needed hacking.
Some meds were used to combat the ill effects of radiation poisoning and a total of 259 stimpacks were consumed to prevent death.

Nerbit ended the killing run on level 30, which is much lower than I thought it would be, though they also maxed out their XP gains, so perhaps that’s why.
Of course, you want to know the total of kills, right?
Well, the total number of creatures killed ended at 970, while the total number of people killed was slightly less at 833.
The video ends with Nerbit blasting their own character with the MIRV weapon - a fat boy launcher that can fire multiple mini-nukes at once, so I suppose you could add that onto people killed.

And it was noted that you can’t kill the kids of the game because they’re immortal, though it should be said this is also a morally grey area, too.
Sadly, there wasn’t any fancy ending or dialogue after they killed everything, but then I guess there wouldn’t be anyone to deliver the final words.
